All research projects utilizing surveys or questionnaires must use the following wording somewhere in the introductory statement on each instrument:

"Participation is voluntary, and you can choose to not answer any question that you do not want to answer, and you can stop at anytime."

[Note: If you are surveying students, you need to include the following: "Your participation or non-participation will not affect your grade or class standing."]
